«They will fall out anyway» is the most expensive piece of advice in paediatric dentistry. Under every baby tooth lies the bud of a permanent one. Inflammation at the root of the baby tooth reaches that bud and damages the enamel of the permanent tooth before it has even erupted — and that cannot be undone later.
The second reason is space. Baby teeth hold the space for the permanent ones. If a tooth is removed two years early, the neighbours tilt into the gap and the permanent tooth has nowhere to go. That is then solved with braces, and costs an order of magnitude more than a filling.
Decay in baby teeth
The enamel of a baby tooth is about half as thick as that of a permanent one, so decay moves faster: from a white spot to pulpitis can take months rather than years. That is why a child is seen every six months, not «when it starts hurting».
We treat it in the same stages as in adults: early decay — no drilling, remineralisation and fluoridation; superficial and moderate — a filling in one visit; complicated — when the infection has reached the nerve, the canal of the baby tooth needs treating. Decay in the front baby teeth
Most often this follows a night-time bottle or a sweet drink before bed: the enamel of the upper front teeth breaks down first. It looks like a dark band near the gum. Here it matters not only to treat but to remove the cause — otherwise you are back in six months with the same problem.
Crowns on baby teeth
When little of the tooth is left a filling does not hold: it falls out within a week and the child goes through treatment a second time. A crown covers the tooth completely and lets it last until it is naturally replaced. It does not look like a «metal tooth» — there are aesthetic options for front teeth.
With or without sedation
Most children are treated calmly with ordinary local anaesthesia — topical gel before the injection, given slowly, with every step explained. Sedation is not needed for a single tooth.
Sedation is considered when the amount of work is large and cannot be done in one visit, when the child is very young, or when there are conditions that make long treatment in the chair impossible. When a child needs an orthodontist
The first check-up is at 6–7 years, when the first permanent molars appear. At that age jaw growth can still be influenced with plates and trainers rather than only moving teeth that have already grown in. The intervention is simpler and cheaper than the same thing ten years later.
What should raise a flag: the child breathes through the mouth, teeth grow in two rows, the jaw shifts sideways on closing, or there was prolonged thumb-sucking. More detail in bite correction in children and the article at what age braces are fitted.
The first visit
It is better to bring a child for the first time when nothing hurts. Such a visit is an introduction: see the room, sit in the chair, count the teeth. The child learns that this place does not hurt and comes back next time without resistance.
Parents stay in the room. We do not promise a child that «it will not hurt» if something is going to be unpleasant — lying means losing trust for good. We have described how the first visit goes separately.
Prices
A consultation with examination and a plan costs UAH 690. Decay treatment starts from UAH 2,350, depending on the size of the lesion. An introductory visit without treatment is arranged separately — call us and we will explain.
The cost of treatment under sedation is calculated individually: it depends on the number of teeth and the anaesthesiologist's time. Quoting a figure before the examination would not be honest.
