Why we rebuild the natural shape

A filling is not just plugging a hole. It restores function, appearance and the balance of forces the tooth will carry for years.

Enamel
The hardest tissue in the body. It does not repair itself. Decay destroys it first — often unnoticed.
Stage 1
Dentine
The sensitive layer beneath the enamel. Decay spreads faster here, and this is where the tubules are — cold and sweet things start to hurt.
Stage 2
Pulp (the nerve)
Once decay reaches here the pain becomes acute. This is pulpitis. Root canal treatment is needed, and the price rises three to five times.

Root canal
The infection enters the canals and then the bone. A cyst, a fistula. The tooth is at risk. The only chance of saving it is good endodontics.

Why a tooth's shape is about more than looks

Every tooth has a unique anatomy: cusps, fissures, contact points with its neighbours. That shape is the product of evolution. It spreads the load of chewing, protects the gums and stops the arch from drifting.

A filling that plugs the hole but does not reproduce the exact anatomy is a time bomb. A wrong contact with the neighbouring tooth traps food, causes decay between the teeth, and creates problems with the bite and the load on the jaw.

Under the microscope the doctor sees detail smaller than a millimetre. That makes it possible to reproduce the natural relief, the contact points and a correct occlusion. The restoration lasts for years — not "until it falls out".

The doctor's rule: remove exactly as much as necessary. Not one extra micron of healthy dentine — a treated tooth should stay as alive as possible. Every micrometre preserved means strength and longevity.

Redoing another dentist's work is always a harder case. Weakened walls, scar tissue, a disturbed microstructure. The quality of our work lowers the risk but does not remove it. That is important to understand.

Nothing hurts — why should I come in?
Decay causes almost no pain until the very end. By the time severe pain appears, it is already pulpitis or the beginning of canal disease. Treatment then costs three to five times as much. The absence of pain is not a sign of health — it is a sign that it is not too late yet.
I am afraid it will hurt
Modern anaesthesia makes dental treatment comfortable. We check that it has worked before starting, and top it up if needed. We never begin until we are sure the patient feels nothing.
My previous dentist said the tooth must come out
Come in for a consultation. Under the microscope we will see the real state of things. Sometimes a tooth that has been condemned can be saved with good endodontics or retreatment. An answer is always better than guesswork.
Will the filling break within a year?
A good restoration made under a microscope from modern materials, properly cared for, lasts five to ten years or more. Fillings fall out because of bonding defects and inaccurate anatomy — precisely what the microscope prevents.
Will the tooth darken after root canal treatment?
Darkening after root canal treatment happens when pulp tissue is left in the canal or outdated materials are used. With good endodontics under a microscope and proper filling, the tooth keeps its natural colour.
