Decay develops gradually. The earlier we step in, the simpler, cheaper and less painful it is. Here is how it progresses.
White spot
The enamel has begun to demineralise. At this stage drilling can still be avoided — remineralisation and fluoride will do.
Superficial
The defect is confined to the enamel. A small filling; the tooth stays alive. There is usually no pain yet.
Moderate
The dentine is affected. Sweet and cold things start to hurt. A full filling is needed.
Deep → pulpitis
The decay has reached the nerve. Severe pain, and root canal treatment is likely — costlier and slower. Better not to get here.

Examination and diagnosis
The doctor examines the tooth under magnification and, if needed, takes an X-ray to see hidden decay and how deep it goes.
Anaesthesia
A modern anaesthetic makes treatment painless. A topical gel goes on first, so even the injection is barely felt.
Removing the decay
Under the microscope we remove only the affected tissue, preserving as much healthy tooth as possible.
The filling, layer by layer
We rebuild the tooth with light-cured composite, layer by layer, matching the shade to your own tooth.
Polishing and the bite
We shape and polish it, then check and adjust the bite so the filling does not interfere and lasts longer. The tooth is done.
