A tablet dulls the pain but does not treat the cause: an inflamed nerve does not settle on its own. While you are on your way to us, here is what genuinely helps and what makes things worse.

  • An ordinary painkiller — strictly as stated on the packaging
  • Cold from outside, 10–15 minutes at a time
  • Sleep with your head raised — lying flat makes the throbbing worse
  • Do not chew on that side and do not provoke the tooth with hot or cold
🚫 Do not
  • Warm the cheek, apply warm compresses or rinses — the inflammation spreads further
  • Place a tablet on the tooth or gum — that is a chemical burn of the mucosa
  • Rinse with alcohol
  • Start antibiotics on your own — they blur the picture for the dentist

An honest approach

First we check whether the tooth can stay

Extraction is a last resort, not a first offer. We work under a microscope and often save teeth that were «due for extraction»: root canal retreatment, microsurgery, rebuilding for a crown.

So the order is this: X-ray → an honest answer on whether the tooth has a prognosis → the decision is yours. If keeping it makes no sense (a root fracture, destruction below gum level, hopeless mobility) we remove it and explain straight away what to do with that space next so the bone does not shrink — the options are on the page about implantation.

The commonest case

Wisdom teeth

A wisdom tooth may lie horizontally, press against its neighbour or never erupt at all. Removing one like that is already surgery: an incision, sometimes sectioning the tooth, and sutures.

A CT scan is mandatory here: the scan shows where the roots point and how close the mandibular nerve runs. Damage to that nerve causes persistent numbness of the lip — which is exactly why such teeth are not removed «by eye».

When removal is justified: the tooth presses on its neighbour, the gum around it is constantly inflamed, a brush cannot reach it and decay is growing there. When it is not: the tooth erupted straight, sits in the bite and causes no trouble — then it is left alone.

When a wisdom tooth is removed and when it is not

Not every wisdom tooth needs removing. The decision is made from a 3D scan, not «just in case».

  • Pressing on the next toothIt pushes against the second molar and slowly destroys it on the side you cannot see in a mirror.
  • Inflammation under the hoodThe tooth has erupted only partly and infection collects under the fold of gum.
  • RetainedIt never erupted and stayed in the bone. It can press on neighbouring roots.
  • Impacted at an angleIt grows at a tilt and shifts the arch, undoing orthodontic work.
  • Decay you cannot reachA brush does not reach there, and treating such a tooth is harder than removing it.
  • No trouble — leave it aloneIt erupted straight, sits in the bite and cleans normally. No need to remove it.

What to do after an extraction

First 2 hours: keep the gauze pad in for 20–30 minutes, do not eat, do not drink anything hot.

First 24 hours: do not warm the site, do not rinse vigorously, do not touch the socket with your tongue — the point is to keep the blood clot, which is the wound's own dressing.

When to call urgently: the pain increases on the third day, the swelling grows, or you have a fever — do not wait, phone us.

What happens to the socket after an extraction

The first two days decide everything: the blood clot has to stay in the socket. It is the dressing under which healing happens.

First hours
Keep the gauze pad in for 20–30 minutes, do not eat for 2 hours. Cold on the cheek for 10–15 minutes at a time.
Day 1–2
No rinsing, no warmth, no smoking. The clot has to stay in the socket — without it dry socket sets in.
Day 3–7
The swelling goes down and the pain eases daily. If it does the opposite, phone us rather than waiting.
Day 7–10
Initial healing. The bone keeps rebuilding for several more months — that is normal and unnoticeable.
