Wisdom Tooth Removal: When It Is Needed, How It Works and What It Costs
Almost everyone has a wisdom tooth ache at least once, and the question is always the same: endure it, wait, or have it out. In short: a wisdom tooth does not need removing just for existing — it is removed when it causes harm. Below is how to tell the two apart, how removal works and why you should not wait until acute pain sets in.
When a wisdom tooth needs to be removed
The trouble with wisdom teeth is that they erupt last, when there is no room left in the jaw. That is where most reasons for removal come from:
Clear-cut indications
The tooth is impacted (has not erupted and lies in the bone) or dystopic (growing at an angle, pressing against the neighbour). The gum flap over it keeps getting inflamed (pericoronitis). The tooth is destroyed by decay a brush cannot reach. The tooth presses on the adjacent second molar and damages its root — visible only on a 3D CT. The front teeth crowd because of it.
When a wisdom tooth can be kept
If it has fully erupted, sits straight and has an antagonist (meets a tooth on the opposing jaw) and can be cleaned properly, there is no point in removing it. Such a tooth can even later serve as a support for a prosthesis. The decision always follows an examination and an image, not a “just in case” approach.
How to tell it is time to see a doctor
Do not wait until it “swells up”. Reasons for an examination include: a dull or sharp pain at the angle of the jaw; swelling or redness of the gum over the tooth; pain on swallowing or difficulty opening wide; a bad taste and smell from that area; pain radiating to the ear or temple. If you are already reading this in pain — do not warm the cheek and do not delay; there is a separate article on the first steps for acute pain: what to do about acute toothache.
How the removal works
It all starts with a 3D CT scan: the image shows the position of the roots and how close the nerve is, so the doctor plans the removal in advance rather than “as they go”. After that it depends on complexity.
Simple extraction
The tooth has erupted and stands in the arch. Under anaesthesia the doctor removes it with an instrument in a few minutes. Recovery is quick, usually with no stitches.
Surgical extraction (impacted / dystopic tooth)
The tooth is in the bone or at an angle. The doctor makes a small access, divides the tooth into parts if needed to remove it without harming the neighbour or the bone, and places stitches. It sounds serious, but under anaesthesia it is painless, and an access planned from the CT heals predictably.
Does wisdom tooth removal hurt
During the removal — no: local anaesthesia is at work, you feel pressure but not pain. Moderate discomfort and swelling are possible for 1–3 days afterwards as the anaesthetic wears off — a normal response to the procedure, relieved by ordinary painkillers as prescribed by the doctor. For anxious patients and complex cases, treatment under general sedation.
What it costs
The cost of removal depends on complexity: a simple extraction of an erupted tooth and the removal of an impacted tooth from the bone are different amounts of work, so the exact figure is quoted after an examination and a 3D CT. A surgeon consultation with an assessment and plan is from UAH 590. At the consultation you get a clear price for your specific case, not a phone “estimate”. All surgical services are on the page surgery and implants.
What you can and cannot do after removal
In the first hours the key is not to disturb the clot in the socket — it protects the wound. So for the first 24 hours: do not rinse vigorously, do not drink through a straw, do not smoke, do not warm the cheek or take a hot bath, and do not exercise. You can eat after 2–3 hours — soft and warm food, chewing on the other side. Swelling is eased with cold applied externally on the first day. If the doctor prescribed an antibiotic, complete the full course.
Frequently asked questions
When must a wisdom tooth definitely be removed?
When it causes harm: impacted or dystopic (lying in the bone or growing at an angle), causing repeated gum inflammation (pericoronitis), destroyed by decay, pressing on the adjacent second molar, or crowding the front teeth. But if the tooth has erupted straight, has a partner on the opposing jaw and cleans normally, it does not need removing. The decision is made after an examination and a 3D CT, not “just in case”.
Does removing a wisdom tooth hurt?
During the removal there is no pain — local anaesthesia is at work and you feel only pressure. Moderate discomfort and swelling are possible for 1–3 days afterwards while the socket heals; this is a normal response, relieved by painkillers as prescribed. For anxious patients and complex removals, treatment under general sedation is available.
How much does wisdom tooth removal cost in Dnipro?
The price depends on complexity: a simple extraction of an erupted tooth and the removal of an impacted tooth from the bone are different amounts of work. So the exact figure is quoted by the doctor after an examination and a 3D CT, once the position of the roots is clear. A surgeon consultation with an assessment and plan is from UAH 590; there you get the price for your specific case.
How long does the socket take to heal after removal?
The soft tissue closes in about 1–2 weeks, the bone remodels longer — a few months — but the discomfort usually passes within a few days. The speed depends on how complex the removal was and on how well you protect the clot in the first day. If on day 3–4 the pain and swelling do not subside but grow, that is a reason to call the doctor.
