A crown came off or chipped: what to do

It usually happens while eating — a sticky sweet, a crust, sometimes for no obvious reason at all. The crown ends up in your hand, the prepared tooth stands exposed, and the first impulse is to find glue. That impulse is the thing to deal with: superglue ruins both the crown and the tooth beneath it, and the cost of the mistake exceeds the cost of the crown.

What to do in the first minutes

1. Take the crown out of your mouth. Carefully — do not swallow or inhale it. If it is swallowed it usually passes naturally, but coughing or breathlessness means seeing a doctor at once.

2. Keep it. Rinse with warm water, no soap and no toothpaste, and put it in a small box or bag. Not in a napkin — that is exactly how crowns get thrown away.

3. Look inside it. Anything dark and hard inside the crown may be a piece of the tooth or a post. Do not pick at it; bring it as it is.

4. Clean the remaining cement off the tooth with a soft brush, no pressure. Keep brushing that area carefully afterwards too: a prepared tooth has no enamel and decays quickly.

5. Do not chew on that side. An exposed core chips easily, and then the crown can no longer go back.

6. Book within days. Not a week, not “after the holiday” — I explain why below.

If the tooth underneath hurts, the gum is swollen, or there is a smell from under the crown, do not wait for an appointment — call. That is no longer about the crown.

Why you should not glue it yourself

Everyone asks, so here it is point by point.

Superglue. The worst option. It is toxic to tissue, locks the crown in the wrong position and makes removal impossible without cutting it off. Most often the crown ends up discarded and the tooth beneath needs retreatment.

Over-the-counter “do it yourself” cements. They exist and are meant for temporary fixing while travelling. The problem is different: without a dentist you cannot see whether the crown seated fully. Half a millimetre short changes your bite, and a week of chewing like that starts hurting the tooth.

The main reason. Crowns do not come off for nothing. Usually something is already happening underneath: decay at the margin, a cracked core, dissolved cement, a loosened post. Gluing over it puts a lid on the problem and lets it grow unseen.

If you are travelling and two days from a dentist. Pharmacy temporary cement is an acceptable compromise. Apply thinly, seat the crown, bite down, remove the excess. And still be seen as soon as you are back.

Why waiting is a bad idea

An empty space in the arch does not stay still.

Neighbouring teeth tip into the gap and the opposing tooth starts to over-erupt. Within a few weeks this is measurable, and the old crown simply will not seat — a new one becomes necessary.

A prepared core has no enamel. It decays several times faster than an ordinary tooth. A month without a crown turns a re-cementation into a treatment.

The core chips. Thin walls without the crown's protection break under normal chewing. A fracture below gum level often means the tooth cannot be saved.

Sensitivity. If the tooth is vital, exposed dentine reacts to cold and to air — simply unpleasant every day.

A realistic window: up to a week with an intact core and no pain. After that the odds of reusing the same crown fall daily.

What happens at the appointment

The dentist assesses three things: the state of the core, the state of the crown, and what the X-ray shows.

Best case — re-cementation. The core is intact, there is no decay, the crown is undamaged. It is cleaned of old cement and seated again. One visit, inexpensive.

The core is partly broken down. It is rebuilt first — a core build-up, from UAH 2,450. Sometimes a post is needed. Then we check whether the old crown still fits: after the core changes shape, often it does not.

Decay or inflammation under the crown. Then treatment comes first, sometimes root canal retreatment, and only then a new crown.

The crown is cracked or chipped. A ceramic chip cannot be repaired reliably, so a new crown is made. That is not greed: bonded ceramic chips again in the same place.

The tooth cannot be saved. This happens when the fracture runs below the gum or the root is cracked. Then we discuss an implant or a bridge — and it is more honest to say so at once than to spend money on a crown that lasts a year.

Prices for a new crown: zirconia from UAH 7,200, eMax from UAH 12,200. A crown on an implant is UAH 16,100 (Megagen) or 18,200 (Straumann). A consultation with a specialist costs UAH 690.

Why it came off, and how to avoid a repeat

The cement dissolved. The commonest and most innocent cause. Cement has a lifespan, and after seven to ten years it gives way. Nobody's fault.

Decay at the crown margin. Plaque collects exactly along the edge and the tooth breaks down beneath it unnoticed. This is the number one avoidable cause.

Too little support. If not much tooth was left, the crown has nothing to hold on to. Sometimes increasing the core height or surgically lengthening the crown portion helps.

Overload. Bruxism and an incorrect bite dislodge crowns regularly. If this is not the first time a crown has come off, the issue is not the cement but the bite, and that is what needs treating.

What makes a crown last: a night guard if you grind; an interdental brush daily; professional hygiene every six months — that is where marginal decay is spotted while still small; and not biting into hard food with the front teeth.
